I would not recommend Southbridge Rehabilitation and Health Center based on my family members recent experience. From the moment he was admitted, he felt dismissed and unsupported by staff. He was in severe pain and prescribed oxycodone, yet he was left waiting for hours without receiving it. When I called to ask about his pain management, the staff was rude and dismissive—not only to me but also to him afterward. No patient should be made to feel like a burden when seeking medical care.

Beyond the poor treatment, I was shocked by the lack of security at this facility. When I arrived to pick him up, I was able to walk right into the building, go all the way to his room, and leave with him—without a single staff member stopping me or checking who I was. That is a massive safety concern for all patients. Even more alarming, his confiscated personal belongings, including prescription medication, were left unattended at the nursing station.

A rehab center should be a place where patients feel safe and cared for. Instead, my partner left feeling neglected, anxious, and deeply disappointed by his experience. I hope the administration takes steps to improve patient care, communication, and security so that others do not have to go through the same thing.

Ann the social worker is amazing, she takes care of any concerns right away and gets the recourses each patient deserves. The CNA’s LUZ and TRACI were amazing and nothing but amazing to my mom during her rehab process and also at the end of her life. The LPNs can be rude and need more training and respectful manner towards patients especially to the patient and family of the dying. A couple of the LPNs were amazing but not all of them. The hospice team was amazing, in making sure my mom was comfortable and was getting all the meds she needed to stay comfortable. I would definitely say there are some Amazing care givers at Southbridge rehab but I definitely think there is still some improvement to be done.

There only concern is what they want , not what the patient actually needs. They will let you suffer before doing the right thing. I personally suffered the 2 nights I was there and I was lied to about the meds I was taking. I was in pain on the 2nd night I was there and wasn't able to sleep for 7 hours before I called emergency services to get me to a hospital to fix the problem they caused. There better places out there, do your research wisely and away from here.

PT staff assisted me in getting to my baseline. However, I wished I received more specifically working on my lower back. I wished we had more activities. My nurse is amazing. The food is excellent I gained a few pounds. Ellie is very caring and knowledgeable always checking in with me to make sure I was doing well. All the staff are great. I appreciate everyone.

My mom was admitted to this facility for a few weeks for physical therapy. The care team here was very attentive to her and communicative with me being all the way in Florida. My mother suffered from a lot of health issues when she arrived, and with the help of this facility was discharged entirely brand new person. I am very grateful for the team here.

The ambassadors are great I really appreciate that Jessica took the time to do my hair and my nails. They also help to resolve problems for example when my clothes were missing they looked for them. No problem is too big for them . Jessica will work on anything presented to her. The social workers are great and the substance abuse counselor is fantastic.The nursing staff is really good, and the mental health treatment is so good.i highly recommend them.

My sister was at Southbridge for almost 6 months on various floors. The staff was very communicative and helpful. It didn't matter if we had a question for the floor nurses, the dietician, Amy in the business office, Jared in the psychology department, or Ann in Social Services, we always got a response and felt that everyone spent time listening and updating us as thoroughly as possible.
I'd like to give an extra special shout out to Ellie Luperon, the Substance Abuse Counselor. She was so incredibly kind to my sister and our entire family. She always responded to questions, followed through with whatever she said she would do, and was my personal lifeline the entire time my sister was there. I honestly felt that Ellie got to know my sister and did everything in her power to help her during her stay there. She went above and beyond every single time we needed her. She also did an amazing job coordinating my sister's discharge and aftercare plan. Words cannot express how appreciated she is and how comforting it was knowing that she was on my sister's care team. Thank you a million times over!

Under staff but they do the best they can. The pt staff are great especially Donovan. Peter the nurse is wonderful and the CNA's Luz, Carmen and the rest of 4th floor work their butt off. Dana is a very hard working. Ellie's groups are great, I attended although I'm a shy person who doesn't like to speak in front of ppl, I felt welcomed and encouraged.¡
